Abstract:

The purpose of this ASRM Practice Committee report is to provide clinicians with principles and strategies for the evaluation and treatment of couples with infertility associated with obesity. This revised document replaces the Practice Committee document titled, “Obesity and reproduction: an educational bulletin,” last published in 2008 (Fertil Steril 2008;90:S21–9).

    Metformin must be combined with a low glycemic diet [not just a low calorie diet] as well as muscle building strength training to see the best results. Our experience has show a dramatic difference in weight loss comparing Glucophage XR vs generic metformin. I’ve not seen comparison studies and weight loss or restoration of ovulation induction in PCOS patients who utilize diet and exercise.

    Weight training that results in muscle building can be far more effective than aerobic exercise. Each pound of muscle burns 75 kcal/day vs 3 kcal/day. Simply adding 6-7 pounds of muscle will have an effect equal to a 500 kcal dietary restriction. Extensive aerobics can result in muscle loss and make it more difficult to lose weight.
